Expanded Chart Awareness
Chart awareness is our AI’s ability to read, understand and reason over the patient's full medical record — not just what was said in the room. It’s the foundation that makes ambient intelligence possible.
Today, we are expanding chart awareness across the following key Ambience features:
Chart-Aware Patient Summary: Clinicians need to see everything about a problem in one place for a patient, without seeking out relevant information across every past note as they prepare for a visit. Chart-aware Patient Summary synthesizes the patient’s medical history into a single view, organized by problem or diagnosis so clinicians can quickly access the context the need to prepare and deliver care with the fullest understanding.
Chart-Aware Assessment & Plan: The A&P is the most clinically significant section of any note. It is also the section most likely to be copy-pasted from a previous visit, creating compliance risk and stale documentation that doesn't reflect the current encounter. With chart-aware A&P, we reduce copy-forward risk by generating a fresh assessment and plan grounded in current visit data and relevant chart history, carrying forward continuity of care while clearly highlighting what changed so the plan always reflects the most up-to-date and relevant information.
Chart-Aware Diagnostics: Adding diagnostic impressions to the note often requires navigating multiple tabs to copy-and-paste, and in this process its easy to lose the right context. When this is multiplied across labs, pathology, and imaging for each patient, chart review can become one of the most draining parts of a clinician’s day. Ambience now extracts, contextualizes and synthesizes trending diagnostics like labs, imaging and pathology into a cohesive view that flows directly into the note, showing the clinical trend and highlighting what’s relevant for today’s encounter.
Chart-Aware Coding: Accurate coding requires more than matching a conversation to a billing code; many codes demand longitudinal chart reasoning to fully capture the complex care delivered. For example, codes like G2211, which captures the complexity of managing chronic conditions over time, can only be substantiated when the AI verifies that a patient has been seen multiple times by the same clinician over an extended period. Similarly, Annual Wellness Visit codes require understanding whether a patient has had a prior AWV to distinguish initial from subsequent billing.
These capabilities join a growing suite of chart-aware features across Ambience. Ambient listening makes note creation faster, but chart awareness makes notes more accurate and clinically relevant by grounding in the patient’s actual complete record. In coding and revenue compliance, our AI co-creates documentation and coding with diagnoses aligned to the way each health system classifies its diagnosis codes, including some of the most complex codes that depend on longitudinal chart reasoning for chronic condition management. And in clinical guidance, chart awareness ensures recommendations are anchored in the patient’s chart, not in clinical generalities.
Together, these capabilities compound. Chart-aware documentation feeds more accurate coding, which drives measurable revenue integrity and establishes the right context for high-quality clinical synthesis — all from the same AI technology layer integrated across the medical record. Expanded chart awareness for Patient Summary and A&P is now live across all health systems where these features are deployed, with chart-aware A&P in beta. Chart-aware Diagnostics is in alpha with select partners and will roll out broadly in the coming months. Early pilots of Chart-Aware Coding are underway with select health system partners. This feature is slated for broad availability in 2026.
